Can using a self-care traction device help and be a cost-effective method of providing cervical relief at home for individuals who have neck pain?

Neck Pain Traction Device

Neck Pain Traction Device: Individuals with neck or arm pain caused by neck/cervical radiculopathy may benefit from physical therapy to manage their condition. (Alshami, A. M., and Bamhair, D. A. 2021) Physical therapy can improve neck mobility, restore function, and decrease pain.

Traction

A physical therapist may use various treatments and modalities to treat the individual's condition. Cervical traction is one treatment to help relieve neck pain and can help:

• Stretch the muscles and soft tissues in the neck.
• Separate and open up the disc and joint spaces.
• Decrease pain in the neck and arms by relieving pressure on nerves from the cervical spine. (Madson, T. J., and Hollman, J. H. 2017)

There are different neck traction techniques. The therapist may use a mechanical traction device that requires the individual to strap their head and neck to a machine that gently pulls the neck, providing relief. (Romeo, A. et al., 2018) The physical therapist may also use manual traction with their hands to help the process. This type allows the therapist to easily adjust the amount of traction force and the direction of the pull on the neck. (Romeo, A. et al., 2018)

Home Traction Devices

Cervical traction that works for individuals at the clinic may also benefit from using a traction device at home to maintain their gains. Over-the-door cervical traction may be an effective, safe, and simple way to administer neck traction in the comfort of one's home to help decrease pain. (Fritz, J. M. et al., 2014) Medical supply stores and pharmacies carry over-the-door traction devices, which can also be found online. If finding an over-the-door traction unit is difficult, the therapy clinic can help order one from a medical supplier.

Setting Up The Device

Before using a neck pain traction device, consult a physical therapist or doctor to ensure neck traction is safe and on usage. Over-the-door traction units may be built differently but comprise the main parts:

• A hook-and-pulley apparatus that hangs over the door.
• A nylon cord that’s fed through the pulley.
• A weight that could be a water bag that hangs on the end of the cord to provide traction force.
• A harness to wear on your head.

1. The traction unit must be hung over a door secured to the top and closed.
2. It is recommended that the door be a closet door so no one will open it while using the device.
3. If a closet door is not available, be sure to lock the door so that no one can open it while attached to the device.
4. A small hook with a pulley attached to it hangs on the door.
5. The harness attaches to the end of the nylon cord opposite the weighted water bag.
6. The harness goes on the head, and the chin strap should fit snugly under the chin and be secured by the hook-and-loop fasteners.
7. The two straps on either side of your head should be hooked to the end of the nylon cord, which is fed through the pulley system.
8. After strapping on the head harness, fill the water bag using the markings on the bag to indicate its weight and hang it on one end of the cord.
9. Fill the bag until the water reaches the desired mark, usually 8–15 pounds.
10. After filling the water bag, sit in a chair facing the door and hang the bag on the other end of the cord not attached to the head harness.
11. Do not drop the bag; this could cause a sudden forceful traction motion on your neck.

How Long to Use

Individuals should generally use the traction device for about 15 to 20 minutes each session and can perform several sessions per day. (American Physical Therapy Association. 2020) While using the over-the-door traction device, there should be a gentle pulling sensation in the neck, relieving the neck pain, and if there is arm pain or tingling, it should also decrease. Traction does not replace neck exercises or postural correction in treating neck pain. Be sure to follow the therapist's prescribed exercises. Active engagement is essential to treating and preventing neck pain from returning. If the pain increases while using the traction device, stop using it and consult a physical therapist or doctor. Ask a physical therapist or other neuromusculoskeletal medical professional if self-care traction is appropriate for specific conditions.

Spinal Rotation

Healthy spine rotation is an important aspect of injury prevention, and rotated vertebrae or a twisted spine can result from spine, nerve, or muscle disease or certain movements.

Normal Spine Twisting Capability

The spine can move in several ways. Spine movements include:

• Bending  - Rounding forward
• Extending - Arching backward
• Tilting sideways is powered by muscles that aid in twisting.

Although the spine can move in many directions, there are limits to how far it can and should go. (Xinhai Shan et al., 2013). This is especially true with twisting. The spinal column is made of 26 interconnected bones called vertebrae. When moving, each vertebrae bone moves accordingly. Rotated or twisted vertebrae, especially when bending forward like lifting heavy objects, are associated with a risk of back injuries like strain and herniated discs.

How Rotation Works

Rotation is a basic movement in which individuals can turn their spinal column. When twisting, the spine also bends to the side. The muscles involved in spine rotation include:

• The internal oblique abdominals and the external oblique abdominals don't directly attach to the spine but are the primary muscles responsible for powering spinal rotation in the lower back.
• Intrinsic muscles, including the multifidus and longissimus, contribute to twisting movement as well.
• The multifidus helps the spine twist when one side is contracted/activated and extends the lumbar spine when both sides contract.
• The multifidus helps control the movement, and the longissimus provides the movement with some extension.

Age and The Spine

As individuals age, the body accumulates tension and/or weakness in the oblique abdominal and other trunk muscles. Sedentary habits primarily bring on these changes. (Pooriput Waongenngarm et al., 2016)

• Chronically tight back and abdominal muscles impair the range of motion of the trunk, as well as twisting ability.
• Muscle weakness and tightness affect spinal movements.
• Weakened muscles can decrease support for spinal movement and decrease overall trunk stability.

Spinal Rotation and Scoliosis

Scoliosis is a common condition that causes a lateral curve of the spine. Some of the vertebrae become displaced to the side. Often, abnormal vertebral rotation underlies this displacement. Treatment often focuses on controlling vertebral rotation with medical guidance and physical therapy. (John P. Horne et al., 2014)

Over-Rotating The Spine

Many individuals over-rotate their spines with manual work, which can increase the risk of back injuries. (National Institutes of Health. 2020). Over-rotation can happen with activities like digging or shoveling.

Exercise For A Healthy Spine

A recommended way to achieve optimal rotation of the spine is with daily back exercises. (National Spine Health Foundation. 2015). An effective back exercise program will consist of movements in every direction.

• Yoga is recommended because it places emphasis on developing flexibility and strength in all directions.
• Pilates does the same.
• An injury prevention exercise program will work the hip and pelvic muscles as well.
• Individuals with a spine condition should consult their healthcare provider or physical therapist about how to exercise the spine safely, as rotation exercises could worsen back problems like bulging or herniated discs.

The Sacrum

The sacrum is a bone shaped like an upside-down triangle located at the base of the spine that helps support the upper body when sitting or standing and provides pelvic girdle flexibility during childbirth. It comprises five vertebrae that fuse during adulthood and connect to the pelvis. This bone takes and endures all of the body's pressure and stress from everyday activities and movements.

Formation

Humans are born with four to six sacral vertebrae. However, fusion does not take place in all sacral vertebrae simultaneously:

• Fusion starts with the S1 and S2.
• As the individual gets older, the overall shape of the sacrum begins to solidify, and the vertebrae fuse into a single structure.
• The process usually starts in the mid-teens and finishes in the early to mid-twenties.
• It is believed to start earlier in females than males.

The timing of the fusion can be used for estimating the age and sex of skeletal remains. (Laura Tobias Gruss, Daniel Schmitt. et al., 2015)

1. The sacrum in a female is wider and shorter and has a more curved top or the pelvic inlet.
2. The male sacrum is longer, narrower, and flatter.

Structure

The sacrum is an irregular bone that makes up the back/posterior third of the pelvic girdle.  There is a ridge across the front/anterior portion of the S1 vertebra known as the sacral promontory. Small holes/foramen on both sides of the sacrum are left over after the vertebrae fuse together. Depending on the number of vertebrae, there can be three to five foramen on each side, though there are usually four. (E. Nastoulis, et al., 2019)

1. Each anterior foramen is typically wider than the posterior or dorsal/backside foramen.
2. Each sacral foramina/plural of foramen provides a channel for the sacral nerves and blood vessels.

• Small ridges develop between each of the fused vertebrae, known as transverse ridges or lines.
• The top of the sacrum is called the base and is connected to the largest and lowest of the lumbar vertebrae - L5.
• The bottom is connected to the tailbone/coccyx, the apex.
• The sacral canal is hollow, runs from the base to the apex, and serves as a channel at the end of the spinal cord.
• The sides of the sacrum connect to the right and left hip/iliac bones. The attachment point is the auricular surface.
• Right behind the auricular surface is the sacral tuberosity, which serves as an attachment area for the ligaments that hold the pelvic girdle together.

Location

The sacrum is at the level of the lower back, just above the intergluteal cleft or where the buttocks split. The cleft starts at around the level of the tailbone or coccyx. The sacrum is curved forward and ends at the coccyx, with the curvature being more pronounced in females than males. It connects to the L5 lumbar vertebra by way of the lumbosacral joint. The disc between these two vertebrae is a common source of low back pain.

1. On either side of the lumbosacral joint are wing-like structures known as the sacral ala, which connect to the iliac bones and form the top of the sacroiliac joint.
2. These wings provide stability and strength for walking and standing.

Anatomical Variations

The most common anatomical variation applies to the number of vertebrae. The most common is five, but anomalies have been documented, including individuals with four or six sacral vertebrae. (E. Nastoulis, et al., 2019)

• Other variations involve the sacrum's surface and curvature, where the curvature differs widely between individuals.
• In some cases, the first and second vertebrae do not fuse and remain separately articulated.
• Failure of the canal to completely close during formation is a condition known as spina bifida.

Function

Studies on the sacrum are ongoing, but some proven functions include:

• It serves as an anchor point for the spinal column to attach to the pelvis.
• It provides stability for the body's core.
• It acts as a platform for the spinal column to rest on when sitting.
• It facilitates childbirth, providing pelvic girdle flexibility.
• It supports upper body weight when sitting or standing.
• It provides extra stability for walking, balance, and mobility.

Conditions

The sacrum can be a main source or focal point for lower back pain. It is estimated that 28% of men and 31.6% of women aged 18 years or older have experienced low back pain in the past three months. (Centers for Disease Control and Prevention. 2020) Conditions that can cause sacrum pain symptoms include.

Sacroiliitis

• This is a common condition of sacroiliac/SI joint inflammation.
• A doctor only makes the diagnosis when all other possible causes of pain have been ruled out, known as a diagnosis of exclusion.
• Sacroiliac joint dysfunction is thought to account for between 15% and 30% of low back pain cases. (Guilherme Barros, Lynn McGrath, Mikhail Gelfenbeyn. 2019)

Chordoma

• This is a type of primary bone cancer.
• About half of all chordomas form in the sacrum, but the tumors can also develop elsewhere in the vertebral column or at the base of the skull. (National Library of Medicine. 2015)

Spina Bifida

• Individuals can be born with conditions that affect the sacrum.
• Spina bifida is a congenital condition that can arise from the malformation of the sacral canal.

Intercostal muscles are the muscles within the rib cage, commonly called the intercostals, which connect the ribs and make up the chest wall. An intercostal muscle strain refers to an injury between two or more ribs. If these muscles become overstretched, restricted, or suffer damage, it can cause inflammation and significant pain in the middle and upper back. Intercostal muscle strain is a common injury in athletes and physically demanding jobs. Chiropractic care and massage therapy can realign the vertebrae with the ribs and loosen and relax the muscles to increase circulation and restore mobility and function.

Intercostal Muscle Strain

The intercostal muscles have different layers attached to the ribs to help build the chest wall and assist in breathing. There are 11 intercostal muscles on each side of the rib cage. Each set is located between connected ribs in the upper and mid-back and consists of the following:

External

• These are the outermost intercostals, responsible for expanding the chest during breathing to help inhale air and allow full deep breaths. 
• The external intercostals originate at the lower edge of a rib and run diagonally to attach to the upper edge of the rib below.
• They are found in the rib cage's back, sides, and front.

Internal

• These sit directly underneath the externals and help collapse the chest during breathing to exhale.
• The muscle fibers run perpendicular to the external intercostals, moving diagonally from front to back along the ribs, and are in the entire rib cage.

Innermost

• These sit directly underneath, run parallel to the internal intercostals, and run from the back of the rib cage to each side.
• The veins, arteries, and nerves lie between the internal and innermost intercostals.

When an intercostal muscle gets twisted, overused, or stretched too far, it can tear, causing muscle strain. Often radiating pain along the rib cage is experienced that extends to the back.

Causes

An intercostal muscle strain often occurs as the result of an injury or overexertion of the muscles. Common causes include:

• Trauma to the rib cage, such as from a fall or automobile collision.
• Impact trauma from sports or physical activities.
• Over twisting the torso beyond its normal range of motion from lifting weights, sports, yoga postures, or dance positions.
• Repeatedly reaching overhead for work or tasks like cleaning or painting.
• Lifting heavy objects above shoulder height.
• Repetitive torso movements.
• A sudden increase in physical activity that the body is not used to can also lead to intercostal muscle strain.
• This can happen when a lack of conditioning or unhealthy postures weaken muscles.

Symptoms

The signs and symptoms can vary, depending on the severity and cause. Symptoms can include:

• Intercostal muscle spasms.
• Mobility difficulties.
• Inflammation, swelling, and sensitivity in the affected area.
• Stiffness and tension, causing upper back pain.
• Upper back and rib pain.
• Tenderness in the area between the ribs.
• Muscle rigidity when bending or twisting the upper body.
• Gradual worsening pain after repetitive movements.
• Worsening pain when coughing, sneezing, or breathing in deeply.
• Severe and sudden pain, particularly if caused by direct trauma to the chest or back.

Chiropractic

Diagnosis involves the individual's medical history and a physical exam to check for movement limitations and assess affected and sensitive areas. Once the inflammation is reduced, chiropractic and physical therapy will focus on the following:

• Pain relief treatment.
• Breathing exercises.
• Posture training.
• Stretching under supervision.
• Strengthing exercises.
• Most cases fully heal within 6 to 8 weeks.

Bruxism is an abnormal jaw clenching or grinding of the teeth, either while awake or during sleep. This can cause neck and shoulder tension caused by excess pressure on the neck and jaw muscles. Individuals may not realize they have bruxism until a dentist notices excess wear and tear or a chiropractor examines their symptoms. Bruxism can play a role in temporomandibular disorders. Bruxism

There is awake bruxism and sleep bruxism. A tight jaw generates tension extending to the neck, shoulder, and upper back muscles. Idiopathic scoliosis means that no cause congenital or neuromuscular that created the spinal deformation has been identified. However, idiopathic scoliosis is the most common type, affecting 2% to 3% of individuals. Individuals diagnosed with an idiopathic disease or condition can be frustrated with more questions than answers, but it can still be treated in adults and kids.

Congenital Scoliosis

• Congenital scoliosis is an abnormal curvature of the spine related to which the patient was born.
• Usually, a failure of formation or segmentation during normal development leads to the spinal condition.

Neuromuscular Scoliosis

• Individuals with neuromuscular scoliosis are usually born with neurological disorders that contribute to muscular unevenness, often resulting in the development of the condition.
• For example, individuals with cerebral palsy are born with muscular unevenness that can contribute to scoliosis development.

Who Is Affected

Anyone can develop scoliosis, but children and adults are divided into separate categories.

Children

• Children with this condition are divided into three subcategories:
• Infantile idiopathic scoliosis
• Juvenile idiopathic scoliosis
• Adolescent idiopathic scoliosis

These classifications are based on age and skeletal maturity.

• Infantile is zero to 3 years old.
• A juvenile is 3 to 10 years old.
• Adolescents are from 11 onward or when starting puberty, to the point where the skeleton fully matures.

Adults

• Idiopathic scoliosis in adults results from undiagnosed or untreated scoliosis in childhood that gradually progressed.

Causes

Research has found a genetic predisposition to developing scoliosis, as it has been found to run in families. Genetic testing has been designed to help determine the risk of developing progressive scoliosis. Theories have been presented about abnormalities that affect the nervous system. These include:

• Dysfunctions of the brain stem or equilibrium have been frequently identified in individuals with idiopathic scoliosis.
• Other theories suggest skeletal growth abnormalities or hormonal/metabolic dysfunction may contribute to the condition.
• However, determining its exact cause remains unknown.

Signs and Symptoms

Here are some things to look out for.

• The body leans to one side.
• There is an unevenness of the ribcage or hips.
• Uneven shoulders.
• Shoulder blades may protrude or stick out.
• The head is not positioned directly above the pelvis.

Diagnosis

Idiopathic scoliosis curves tend to follow predictable patterns.

• Right thoracic or middle back scoliosis
• Left thoracolumbar or mid and low-back scoliosis
• Relative thoracic hyper or hypo kyphosis

Magnetic resonance images/MRIs of the spine can show evidence of any significant abnormalities. If no other related condition is present to suggest different causes, then the idiopathic scoliosis diagnosis can be made.

Treatment

Treatment depends on the individual's age and the degree of curvature in the spine.

• In many cases, adolescent or juvenile idiopathic scoliosis patients that have a mild curve can be treated with a brace.
• Adults could need surgical intervention, such as a fusion surgery where rods and screws are added to realign the spine and relieve the pressure on the nerves.

Music therapy, combined with standard medical treatment has been found to reduce the perception of pain in individuals recovering from spinal surgeries, according to a study published in the American Journal of Orthopedics. Spine surgery recovery can take a long time and take a tremendous toll on the individual and family. Research has found that music therapy can help reduce pain post-surgery.

The study has shown solid findings that this therapy, when combined with standard treatment, positively impacts pain. Individuals report an increase in comfort during the recovery time. Utilizing music in a therapeutic setting can benefit a patient's treatment plan, as it addresses the whole individual, including mind, body, and spirit. This is the same approach that chiropractic uses.

Combining Standard Treatment with Music Therapy

The study took place at the Spine Institute of New York with the music therapy set-up through the hospital’s Louis Armstrong Center for Music and Medicine. There were 60 patients thirty-five females and twenty-five males ranging in age from 40 to 55. They underwent anterior, posterior, or anterior-posterior spinal fusion surgery. The groups were equally split up.

The experimental group had music therapy included with their standard medical care, and the control group only received standard medical treatment.

The experimental group would undergo a 30-minute music therapy session over an 8-hour period within 72 hours after the procedure. The music options included:

• Patient-preferred music
• Singing
• Rhythmic drumming that encourages relaxation

The sessions focused on personal treatment, where the individuals were encouraged to express emotions. A pain scale was used before and after the interventions to measure the results. What was found, was that pain levels would go up slightly in the control group, and in the experimental group, the pain dropped.

Spine Surgery Recovery

With standard care medications are typically the first-line treatment post-spine surgery pain. With all of the negative side effects that come with pain meds/opioids, music therapy is worth considering when looking for non-drug therapies. If anything music is an excellent distraction tool. So long as the music is enjoyable to the individual.

For the most part, many individuals that underwent spine surgery already listen to their music pre and post-operation. It helped them to relax before the surgery and allowed them to focus on something else after. Much like athletes that have their headphones/earbuds when they get to the venue to focus and block out everything going on around them. This therapy works the same way. But now we want to encourage anyone undergoing some spinal procedure to go ahead and jam out.

Chiropractic Massage Therapy

Spinal stenosis is the term used to describe a narrowing spine. Treatments vary because everybody's case is different. Some individuals experience mild symptoms, while others experience severe symptoms. Can knowing treatment options help the patient and healthcare team customize and personalize a treatment plan to the individual's condition?

Spinal Stenosis Treatments

Spaces within the spine can become narrower than they're supposed to be, which can cause pressure on nerve roots and the spinal cord. Anywhere along the spine can be affected. The narrowing can cause pain, burning, and/or aching in the back and weakness in the legs and feet. Spinal stenosis has several primary treatments. When working through spinal stenosis treatments, a healthcare provider will assess symptoms and start treatment with first-line therapy, such as pain medication and/or physical therapy. These are often the first among individuals with the disease.

Medication

Chronic pain is one of the main symptoms. The first-line treatment often involves using pain-relieving medication/s. Commonly prescribed medications are nonsteroidal anti-inflammatories or NSAIDs. These medications reduce pain and inflammation. However, NSAIDs are not recommended for long-term use, and other medications may need to be used to relieve pain that includes: (Sudhir Diwan et al., 2019)

• Tylenol - acetaminophen
• Gabapentin
• Pregabalin
• Opioids for severe cases

Exercise

Exercise can reduce spinal stenosis symptoms by taking pressure off the nerves, which can reduce pain and improve mobility. (Andrée-Anne Marchand et al., 2021) Healthcare providers will recommend the most effective exercises for the individual. Examples include:

• Aerobic exercises, such as walking
• Seated lumbar flexion
• Lumbar flexion in lying
• Sustained lumbar extension
• Hip and core strengthening
• Standing lumbar flexion

Physical Therapy

Another primary spinal stenosis treatment is physical therapy, which is often used alongside pain medications. Typically, individuals undergo six to eight weeks of physical therapy, with sessions two to three times weekly. Utilizing physical therapy has been shown to (Sudhir Diwan et al., 2019)

• Reduce pain
• Increase mobility
• Reduce pain medications.
• Reduce mental health symptoms like anger, depression, and mood changes.
• For severe cases, physical therapy following surgery can reduce recovery times.

Back Braces

Back braces can help reduce movement and pressure on the spine. This is helpful because even small spinal movements can lead to nerve irritation, pain, and worsening symptoms. Over time, the bracing can lead to a positive increase in mobility. (Carlo Ammendolia et al., 2019)

Injections

Epidural steroid injections may be recommended to relieve severe symptoms. Steroids act as anti-inflammatories to reduce pain and swelling caused by inflammation and irritation of the spinal nerves. They are considered nonsurgical medical procedures. According to research, injections can effectively manage pain for two weeks and up to six months, and some research has found that after a spinal injection, relief can last 24 months. (Sudhir Diwan et al., 2019)

Thickened Ligaments Decompression Procedure

Some individuals may be recommended to undergo a decompression procedure. This procedure involves using a thin needle tool inserted into the back. The thickened ligament tissue is removed to reduce the pressure on the spine and nerves. Research has found that the procedure can reduce symptoms and the need for more invasive surgery. (Nagy Mekhail et al., 2021)

Alternative Treatments

In addition to first-line treatments, individuals may be referred to alternative therapies for symptom management, including:

Acupuncture

• This involves the insertion of thin-tipped needles into various acupoints to relieve symptoms.
• Some research has found that acupuncture may be more effective at reducing symptoms than physical therapy alone. Both options are viable and can improve mobility and pain. (Hiroyuki Oka et al., 2018)

Chiropractic

• This therapy reduces pressure on nerves, maintains spinal alignment, and helps to improve mobility.

Massage

• Massage helps to increase circulation, relax the muscles, and reduce pain and stiffness.

New Treatment Options

As spinal stenosis research continues, new therapies are emerging to help relieve and manage symptoms in individuals who don’t respond to traditional medicine or cannot partake in conventional therapies for various reasons. However, some evidence presented is promising; medical insurers may consider them experimental and not offer coverage until their safety has been proven. Some new treatments include:

Acupotomy

Acupotomy is a form of acupuncture that uses thin needles with a small, flat, scalpel-type tip to relieve tension in painful areas. Research on its effects is still limited, but preliminary data shows it could be an effective complementary treatment. (Ji Hoon Han et al., 2021)

Stem Cell Therapy

Stem cells are the cells from which all other cells originate. They act as the raw material for the body to create specialized cells with specific functions. (National Institutes of Health. 2016)

• Individuals with spinal stenosis can develop soft tissue damage.
• Stem cell therapy uses stem cells to help repair injured or diseased tissues.
• Stem cell therapy can help repair or improve the damaged areas and provide symptom relief.
• Clinical studies for spinal stenosis report that it could be a viable treatment option for some.
• However, more research is needed to confirm whether the therapy is effective enough to be widely used. (Hideki Sudo et al., 2023)

Dynamic Stabilization Devices

LimiFlex is a medical device undergoing research and analysis for its ability to restore mobility and stability in the spine. It is implanted into the back through a surgical procedure. According to research, individuals with spinal stenosis who receive the LimiFlex often experience a higher reduction in pain and symptoms than with other forms of treatment. (T Jansen et al., 2015)

Lumbar Interspinous Distraction Decompression

Lumbar interspinous distraction decompression is another surgical procedure for spinal stenosis. The surgery is performed with an incision above the spine and places a device between two vertebrae to create space. This reduces movement and pressure on the nerves. Preliminary results show positive short-term relief from symptoms; long-term data is not yet available as it is a relatively new spinal stenosis treatment option. (UK National Health Service, 2022)

Surgical Procedures

There are several surgical procedures are available for spinal stenosis. Some include: (NYU Langone Health. 2024) Surgery for spinal stenosis is often reserved for individuals with severe symptoms, like numbness in the arms or legs. When these symptoms develop, it indicates a more notable compression of the spinal nerves and the need for a more invasive treatment. (NYU Langone Health. 2024)

Laminectomy

• A laminectomy removes part or all of the lamina, the vertebral bone covering the spinal canal.
• The procedure is designed to reduce pressure on nerves and the spinal cord.

Laminotomy and Foraminotomy

• Both surgeries are used if an individual's spinal stenosis negatively affects an opening in the vertebral foramen.
• Ligaments, cartilage, or other tissues that constrict the nerves are removed.
• Both reduce pressure on the nerves traveling through the foramen.

Laminoplasty

• A laminoplasty relieves pressure on the spinal cord by removing parts of the spinal canal's lamina.
• This enlarges the spinal canal and relieves pressure on the nerves. (Columbia Neurosurgery, 2024)

Discectomy

• This surgical procedure involves removing herniated or bulging discs that are placing pressure on the spinal cord and nerves.

Spinal fusion

• Spinal fusion involves joining two vertebrae using metal pieces like rods and screws.
• The vertebrae are more stable because the rods and screws act as a brace.

Which Treatment Is The Right One?

Because all treatment plans differ, determining the most effective is best suited for a healthcare provider. Each approach will be personalized to the individual. Individuals with neck and shoulder pain may experience what feels like tightened lumps or knots in and around the muscles where the neck and shoulder meet. Can using kinesiology tape for neck and shoulder trigger points help to loosen and release them, restore function, and bring pain relief?

Kinesiology Tape For Neck and Shoulder Trigger Points

The upper trapezius and levator scapula muscles are where the shoulder and neck come together and are often the location of trigger point formations. These trigger points can cause tension, pain, and muscular spasms in the neck and shoulders. Various treatments for releasing trigger points and alleviating the pain symptoms include therapeutic massage, trigger point release, and chiropractic adjustments in a multidisciplinary treatment approach.

• Electrical stimulation and ultrasound have often been used to break up the knots, but scientific evidence has shown that these treatments alone are not the most effective. (David O. Draper et al., 2010)
• Stretching the neck muscles can bring tension relief and help release the knots.
• Practicing healthy postures helps avoid and prevent symptoms. (Cleveland Clinic. 2019)
• Kinesiology tape can decrease pain and spasms and help to release the trigger points.

Therapy

Using kinesiology tape is a form of physical therapy that can be used in various ways.

• The tape helps lift the upper tissues from underlying tissues to increase circulation and release muscular spasms.
• It can help improve muscular contractions, decrease swelling, and inhibit pain in injured tissues.
• Helps stop the trigger points and knots from worsening.
• The tape can also be used for managing lymphedema.

Usage

To decrease trigger points, individuals can use a specific kinesiology tape strip called a lift strip. Individuals can consult their healthcare provider or physical therapist to show them the various types of strips to learn how to cut them properly.

• Before using kinesiology tape, consult a healthcare provider or physical therapist to assess the injury and situation.
• Kinesiology tape is not for everyone, and some people have conditions where the use of kinesiology tape should be avoided altogether.
• A therapist can evaluate the neck pain and trigger points to determine if the individual should use kinesiology tape.

To use kinesiology tape for neck and shoulder trigger points:

1. Get comfortable with the neck and shoulders exposed.
2. Cut one lift strip for each side of the neck, if necessary.
3. The lift strip should be around 3 to 4 inches long.
4. Remove the paper backing in the center with the exposed tape in the center, which should look like a band-aid.
5. Both ends of the lift strip should still have the paper backing on.
6. Stretch out the kinesiology tape.
7. Place the stretched tape directly over the trigger points in the upper shoulder area.
8. Remove the backing on either side of the lift strip and place the ends on without stretching.
9. Gently rub the tape to help the adhesive adhere.

• Once the tape has been applied, it can be left there for 2 to 5 days.
• It's ok if it gets wet with a bath or shower.
• Monitor the skin around the tape to watch for redness or other signs of a negative reaction to the tape.
• Kinesiology taping can be a useful tool to decrease pain and spasms but does not replace professional treatment, prescribed exercises and stretches, and posture retraining.
• The physical therapy team will teach proper self-care strategies for the individual's condition.
• For individuals with neck and shoulder pain and muscle spasms, a trial of kinesiology taping may help alleviate symptoms and improve the overall injury.
