Herniated, slipped, or ruptured discs affect 80% or more of the population. Most individuals don't even realize they suffered a vertebral subluxation, as it shifted slightly but returned on its own and healed itself. Herniated disc/s symptoms can subside over time and can heal on their own. However, there are times when chiropractic is necessary to help the slipped or ruptured disc back into correct alignment and to help prevent re-injury or the development of new ones.

When Chiropractic Is Necessary

When an individual's ability to move is limited is definitely when chiropractic is necessary. Individuals twist and turn their bodies, and the rotational force that comes from lifting and moving objects at home, work, school, sports, or lifting weights increases the risk of disc injury.

• The lumbar spine or lower back is the most common location for a herniated disc injury.
• The pain can spread to the glutes and legs, causing sciatica or sciatica-like symptoms.
• When back pain spreads to the shoulder through the arm, it s caused by a herniated neck/cervical disc.
• When the cushioning material from the disc/nucleus pulposus presses on surrounding nerves, it causes inflammation, pain, and numbness.
• Individuals can suffer a herniated disc after changing a flat tire, stepping/slipping out of the bath/shower, or coughing and sneezing.

Healing

Herniated discs can be treated with ice packs and heat, over-the-counter medications, and anti-inflammatories. However, if these approaches are not producing results, chiropractic and physical therapy could be necessary to address the pain, reactivate the body's healing system, and get the body's circulation energy flowing. Exercises/movements are recommended depending on the injury to allow the musculoskeletal system to realign and circulate the nutrient-rich blood.

Evaluation

The chiropractic team must check if the individual is cleared for chiropractic care. Some individuals cannot undergo chiropractic adjustments because of the following:

• Spinal cancer
• Advanced osteoporosis
• Bone abnormality in or around the upper neck
• High risk of stroke

Once The Patient Is Cleared:

• The chiropractor will assess the injury and damage by evaluating the spine's overall health, not just the painful areas.
• They will inquire about medical history and conduct a physical examination.
• Diagnostic tests could be necessary depending on the condition.

The team will evaluate the following criteria:

• If reflexes are normal.
• If there is muscle loss or decreased muscle strength.
• If there is numbness or loss of sensation.
• Loss of reflexes, muscle strength, and sensation could indicate the need for more aggressive treatment.

Depending on what is found, they may refer the individual to a spinal surgeon or specialist.

Techniques

Chiropractic focuses on restoring structural integrity to the body, reducing pressure on neurological tissue, and re-establishing a normal range of motion. With this treatment, pain and inflammation will be reduced or eliminated, and regular movement and reflexes will return. The body is realigned, stress is reduced, and the body's natural energy can repair the damage. Adjustments involve:

• HVLA is a high velocity, low amplitude short thrust to vertebrae that are out of position.
• Mobilization involves low-velocity manipulation, stretching, and moving affected muscles and joints.
• Joint cavitation expels oxygen, nitrogen, and carbon dioxide from the vertebrae and releases pressure on the affected area.

Specialized Adjustment Techniques

Activator

• Activator Adjusting Instruments emit mechanical low-force, high-speed impulses to the body.

Active Release Technique

• This soft tissue technique treats issues with muscles, nerves, ligaments, tendons, and fascia.

Cox Flexion Distraction

• Special tables with movable segments gently stretch and decompress spinal facets and ligaments.

Diversified

• These are specific manual thrusts to restore biomechanical function.

Gonstead Technique

• This specific analysis method uses nervoscopes, spinal X-rays, and precision adjustments.

The patient will be asked to fill out forms that can also be filled out online before the visit, saving a patient time if they so choose. This provides background information about symptoms and conditions in preparation for the in-person chiropractic consultation. Questions include:

Diagnostic Studies

Heart Disease

Many factors can contribute to heart disease, and research has pointed to inflammation caused by obesity as one of the most significant factors to its development. The main contributor is cytokines produced by excess body fat. Cytokines cause inflammation in the walls of the arteries causing damage and increasing pressure. Blood pressure is the force of blood pushing against the walls of the vessels. High blood pressure means that the heart is not pumping blood effectively, and the heart begins to enlarge. An enlarged heart is a significant risk factor that can lead to heart failure if interventions are not taken to repair the damage.

Ladies, ever wonder why you suffer from regular bouts of lower back pain?  Achy hips?  How about crazy tight leg muscles?  Don’t blame it on your cycling class, or too many squats or, the trainer you only see once or twice a week.  Look down.  Are you wearing high heels?  Bingo!  You’ve heard high heels are bad for you.  But it’s not just because they cause all kinds of pain and trauma to your feet.  High heels are also messing up your physical fitness.  They throw you out of proper postural alignment causing your joints and spine to take on more wear and tear, which means aches and pains.

Is it possible to still look rockin’ and save your joints?  My suggestion is more Athleisure-wear.  I know some fashion hard-liners say, no way will I walk around in yoga pants on a weekday!  But we’ve come a long way since those flare-leg, fold-over yoga pants.

Let’s chat for a moment about the evils of high heels.
First there’s the obvious.  They make your feet hurt.  Blisters, calluses and swelling are par for the course.  And pointy toes, fuhgeddaboutit!  I’m sure they were invented by someone on the Marquis de Sade’s payroll.  Second, they can lead to foot injuries like plantar fasciitis (usually from a bone spur that makes your heels hurt), hammertoes, bunions, and neuromas.   Then there’s the domino effect.

Not only do high heels make your feet hurt, but problems with the feet can travel up the leg and cause injuries in the back, knees and hips.  Your knees take on extra pressure from the weight being pushed forward onto the balls of the feet.  Walking in this position makes your hip flexors and calf muscles short and tight.  And it doesn’t stop there.  Back problems are incredibly common in women who don’t give up their high heels.

Here’s why:

1. Postural changes: The S-curve of your spine has cushiony discs in between the vertebrae that act as a shock absorbers to protect them from stress. Like when you’re bending or jumping.  Wearing heels causes the lower back to arch more than normal because the body weight is pushed forward.  To compensate, the upper body has to lean back to maintain balance.  This puts extra stress on the discs.   Spending hours with your body in funky alignment can lead to muscle spasms and back pain.  Tight hamstring muscles, which attach to back of the pelvis and lower back, can also make your back ache.
2. Anatomical changes: Wearing high heels on a daily, or very regular basis, over years, can actually cause anatomical changes to your body.  In addition to the extra strain on your back and knees, the calf muscles can also shorten and the tendons can get tighter and thicker.

And It Can Get Even Nastier If You Get One Of These Spine Injuries:

• Spondylolisthesis: it’s a mouthful, but is a common injury that can happen in the lower back from too much hyperextension (arching the back).  It’s when one vertebra slips forward over another.
• Foraminal stenosis: I have this one congenitally and it sucks. This is a spine and nerve issue that occurs when anatomical abnormalities reduce the spaces the nerves travel through as they exit the spinal column.  The spaces are called foramina, and when they get blocked, the nerves get squeezed.  The pain can radiate through the buttocks and down the legs.  Symptoms are shooting pains, numbness, tingling, muscle weakness, spasms and, or cramping.
• Sciatica:  The sciatic nerve is the longest one in the body.  It runs from the bottom of the lumbar spine all the ways down the legs.  When the sciatic nerve gets compressed it causes radiating pain, tingling, numbness and muscle weakness down the leg and can The pain can travel all the way to the bottom of the foot.

What Can You Do?  My Case for Athleisure-Wear To Combat Back Pain
  
It’s time for stuffy office attire to retire.  Comfy clothes and flat shoes can be very chic.  Have you seen the boards on Pinterest?  Thanks to this hopefully permanent fashion style, my jeans and heels spend more time in my closet than on my body.  Leggings and cute kicks are my go to’s.  Night out?  No prob.  I reach for my sleek workout leggings, a ruched top or off the shoulder top with some high heel sneaks (they’re wedges so not nearly as bad for you).  I also discovered these by Bluprint which I put to the test at 2 huge conventions where I walking and standing for hours on end.  The soles made of memory foam – like those beds!
 
My podiatrist friend, Steven Rosenberg, DPM has been preaching the need for comfy shoes to his female clients for years.  (Fortunately for his practice, not everyone listens!)  Dr. Steve says, wearing shoes designed more for comfort can help you live more pain-free.  “Because comfort shoes are made of soft cushiony materials with soft foam innersoles, those are what you should turn to for shopping, walking or standing for long periods of time to avoid blisters, muscle spasms or arch cramps.”   He also says to check for arch support in your shoes.  If there’s none, you can buy ones to put inside.
 
Even after reading this, you may still not be willing to give up high heels for good.  Me either.  I still get glammed up once in a while.

Here Are Some Tips For When You Must:

• Wear them for as little time as possible.
• Try to opt for heels around 2” high
• Steer clear of pointy toes.
• Buy shoes with leather insoles to so your foot doesn’t slide.
• Buy arch inserts or use orthotics to support your arches.
• Vary your footwear so you’re not wearing high heels every day.
• Gradual or lower slopes are a little better, go for platforms or wedges instead of stilettos
• Thicker heels are better than spiky heels
• Stretch and strengthen the overworked muscles.

Here’s How:

• Stretch your leg muscles and hip flexors before and after wearing heels.  For the calves, stand on a step and let one heel hang down until you feel a stretch.

Try this convenient device, the foot rocker by Vive.  It stretches the calf and the sole of the foot relieving pain from plantar fasciitis.

• Massage and stretch the muscles in the soles of your feet.  Roll your foot on a golf ball before and after wearing heels and, get regular foot massages.
• Strengthen and increase the range of motion in your ankles and feet.  Put a rag on the floor.  Using your foot, write the letters of the alphabet.  Also put a bunch of marbles on the floor.  Pick one or a few of them up using just your foot.  Put them down about 6 inches away without lifting your foot off the floor.  Try that 10 times.  If you don’t have marbles, you can do it with a hand towel.

So next time you’re shopping for shoes, think about your foot fitness first.  Look for fashion that keeps you closer to the ground and that will hopefully keep you farther away from the doctor.

A variety of issues weigh in on an individual's likelihood of ending up with sciatica. Most of them deal with increased pressure on the spine.

According to the Mayo Clinic, sciatica can best be described as a compression of the nerves on the spine, most commonly occurring as a result of a herniated disk or a bone spur. The impingement of the nerves can then cause inflammation, pain, and often, symptoms of tingling and numbness on the affected leg. Sciatica can commonly be the result of increased stress on the spine and/or nerves of the spine. 

Obesity, improper lifting, a sedentary lifestyle, and getting older are all frequent factors that cause sciatica and its familiar symptoms. Carrying too much weight can strain the spine, causing damage that may result in sciatica just as twisting the body incorrectly to lift heavy objects can also result in injury to the spine.

Treatment options for sciatica can vary and the choice depends on the severity of the condition.

Chiropractic care for sciatica works to align the spine and reduce the pressure on the lower back. Together with strengthening exercises, a person's symptoms of sciatica can be greatly reduced. Acupuncture has also shown positive results in the treatment of sciatica. Other treatment options for sciatica involving medication include, anti-inflammatory drug therapy, supplements, and cortisone injections. 

Introduction

In the lower half region of the body, a large nerve connects to the lumbar region of the spine that travels down to the legs. This is the sciatic nerve, and its primary function is to provide mobility to the legs. The legs use the sciatic nerve to move, feel, and stabilize the body as it is in motion. What Is Sciatica?

Do you feel pain running down your legs? Does it hurt when you are walking, even for a short distance? How about muscle stiffness that is occurring in your lower back and legs? All these symptoms are signs that you are experiencing sciatica. Research studies have defined sciatica as a throbbing, sharp pain that affects one side of the body and travels down each leg. Scoliosis causes the spinal curvature to go sideways and can cause various symptoms. One of the most common symptoms is scoliosis hip pain. This happens because the spine's curvature can pull the hips out of alignment, causing one side to be raised higher than the other. A result of this is one hip begins to take on more weight. This overloads the hip causing strain, soreness, aches, and pain, especially after standing or walking.

Scoliosis Hip Pain

Idiopathic scoliosis typically occurs in children, but often there are no pain symptoms. This type of scoliosis can go on unnoticed and undiagnosed for years even until adulthood. When the spine stops growing, even a small scoliosis curve can cause hip pain. The hip pain varies but generally presents with:

• Sharp pain.
• Throbbing pain.
• Pain that increases with activity like walking, running, standing for a long time.
• Stiffness with combined pain after sitting or lying down for a long time.
• Sciatica.
• Numbness.
• Tingling.
• Tight hip flexors.

The amount of pain depends on:

• The misalignment severity.
• The type of scoliosis - degenerative or idiopathic.
• Bodyweight. Individuals that are obese are more likely to have severe hip pain because of the added weight.
• Activity level. Physical activity can cause the pain to worsen.
• Lifestyle.
• It varies from person to person.

The Higher Hip

When scoliosis has progressed enough, the curvature and uneven hips are pretty apparent. However, only around 10% of scoliosis cases get this far. If the curvature is minimal, which is usually the case, trying to determine which hip is higher usually requires medical assistance.

• The hip that presents with pain is generally the one taking on more weight.
• This causes the muscles to fatigue faster and places added stress on that hip joint.
• A way to tell which hip is higher stand on bare feet in front of a mirror holding a piece of yarn, or string with both hands.
• Place the heel of each hand on the corresponding hip bone.
• The string will be tilted if the hips are uneven or will be straight if they are not.

Adult Scoliosis 

Hip pain caused by scoliosis is most common in adults. However, it is not the only cause of hip pain. Unless diagnosed with scoliosis as a child, individuals may be suffering from de novo scoliosis or degenerative scoliosis.

De Novo/Degenerative Scoliosis

As the body ages, the spinal discs between the vertebrae begin to wear down. The worn-down discs can cause the spine to develop a sideways curve, that can pull the hips out of alignment. Studies are showing that this type of scoliosis is widespread. Loss of bone density can be a contributor to scoliosis in older adults. Bone density loss from menopause means women are more susceptible than men. Past the age of 70, both men and women lose on average, the same amount of bone density. There are effective integrative, natural treatment therapies that incorporate:

• Exercises
• Nutrition
• Posture correction
• Chiropractic
• Physical therapy

Scoliosis Hip Exercises

For uneven hips, there are exercises to help relieve pain and strengthen weak muscles.

Hip Stretch

• Lie on the back with both legs straight out.
• Lift the right leg
• Using the hands pull the knee toward you, keeping the foot pointed up.
• Pull the knee to your chest as far as you can without causing discomfort or pain.
• Hold for 5 to 8 seconds.
• Release.
• Switch to the other leg.
• Repeat 4 to 6 times on each leg.

Wide Leg Stretch

• Sit on the floor with the legs as wide as they can go.
• With the right hand, reach toward the left foot, touching if possible.
• Return to the original position.
• With the left hand, reach toward the right foot.
• Repeat 6 to 8 times.

Chiropractic

If misaligned and uneven hips are causing pain, chiropractic care for scoliosis is recommended. The Foundation for Chiropractic Progress (F4CP) notes that for more than 10 years, the U.S. Department of Veterans Affairs (VA) has included chiropractic services as part of the standard medical benefits package offered to all enrolled veterans. According to a new study conducted by researchers from the VA Connecticut Healthcare System, published in the Journal of Manipulative and Physiological Therapeutics, the use of chiropractic services and the chiropractic workforce in VA has grown substantially since their introduction over a decade ago.

“Our work shows that VA has steadily and substantially increased its use of chiropractic services each year following their implementation in late 2004,” states lead author of the study Anthony J. Lisi, DC, Director of the VA Chiropractic Program, and Chiropractic Section Chief at the VA Connecticut Healthcare System. He then added, "VA continues its efforts to ensure appropriate access to chiropractic care across the whole system, but as this paper shows, the progress to date has been remarkable.”
